How the official source says to file

• Use the electronic intake linked from the official complaint page, or mail its paper form as instructed. To request a form, call 800-201-9353.
• Electronic intake requires email and phone verification. Prepare the practitioner's full name, practice address, treatment dates and locations, and a factual account of the concern.
• The paper form asks whether supporting records exist; follow TMB's instructions for providing them. Read the declarations before signing.

What this route may not provide

TMB may take corrective or disciplinary action but does not award injury damages. For physicians, §164.206 permits a refund in an agreement resulting from an informal settlement conference, capped at what the consumer paid the license holder for the regulated service. Filing does not guarantee repayment.

Do not guess about confidentiality

Anonymous complaints are not accepted. Identity is generally protected, with exceptions. The paper form warns that referrals to another agency include your complaint and identifying information; the receiving agency's confidentiality rules apply. Do not assume a practitioner cannot infer who complained.

Physician-care complaints: seven-year rule, with exceptions

For physician care, TMB generally cannot act if it receives the complaint more than seven years after the care. For care provided to a minor, the limit is the later of the patient's 21st birthday or seven years after the care. A narrow exception for alleged assisted reproduction knowingly using donor material without the patient's express consent to that donor allows the later of seven years after the occurrence or two years after the complainant knew or should have known the facts (§154.051(d-1)). This is not a general sexual-misconduct exception. Other license types' timing was not verified; ask TMB promptly. These are Board-complaint time limits, not civil-lawsuit deadlines.

What may happen after filing

TMB first assesses jurisdiction and whether a possible violation warrants further review. Investigation may include medical records and expert review. Cases can close without action or lead to corrective or disciplinary measures; published timing estimates do not guarantee a completion date.
