DOPL maintains physician and surgeon, osteopathic physician and surgeon, and physician-assistant licensing programs. Each program’s current Board Members page names the same Medical Licensing Board, with MD, DO and PA members. These credentials use DOPL’s professional complaint intake. Identify the practitioner’s actual license; a complaint about a hospital or insurer is not automatically a complaint against an individual professional.

How the official source says to file
- Start with File a Complaint Online on DOPL’s complaint page, or use its manual complaint form. Complete every applicable section and mark inapplicable sections N/A.
- Provide the individual’s profession, license number if known, location and dates, witness information, your relationship, and a detailed chronology. Attach copies of supporting evidence; originals and other submitted materials will not be returned.
- The paper form directs delivery to DOPL, Attn: Investigations, Heber M. Wells Building, 1st Floor, 160 East 300 South, PO Box 146741, Salt Lake City, UT 84114-6741. Read its declaration, identify yourself or your authority to represent the complainant, and complete the signature/date information. Call 801-530-6628 for assistance.

What this route may not provide
DOPL enforces professional licensing laws. It does not act as your attorney, settle your dispute, recover monetary losses, or investigate for the benefit of a private lawsuit. Filing does not establish entitlement to a refund or compensation.

Do not guess about confidentiality
The webpage describes complaints as nonpublic. The paper form separately authorizes disclosure of the complaint and evidence for investigation and makes the records subject to Utah records law. Anonymous acceptance and concealment of identity from the practitioner were not established; ask DOPL before relying on either.

A filing deadline was not established by this check.
Ask DOPL promptly about the dates and credential involved. The checked instructions provide neither an incident-to-filing cutoff nor a guaranteed completion time. A licensing complaint should not be assumed to preserve a separate court claim.
What may happen after filing
DOPL reviews the complaint and says it will notify you what action it can take. The form requires cooperation and warns that insufficient cooperation can cause an investigation to close. Further license-specific investigative stages were not established by this check.
