DOPL lists pharmacists, pharmacy interns, technicians and trainees, pharmacies, remote dispensing pharmacies and specified dispensing practices or clinics. Identify the person and business, their Utah credentials if known, and where the conduct occurred; ask DOPL to confirm jurisdiction for a mail-order transaction.

How the official source says to file
- Use the online complaint link or the manual form. Complete every section or mark it not applicable. Provide individual and company names, profession, license number if known, location, dates, witnesses and a factual timeline.
- Attach supporting copies such as receipts, correspondence, photographs and records. Do not send originals: materials will not be returned. Complete the dated truthfulness declaration and confirm that you are the complainant or an authorized representative.
- The manual form directs filing to DOPL, Attn: Investigations, 160 East 300 South, P.O. Box 146741, Salt Lake City, UT 84114-6741. Ask 801-530-6628 about delivery questions and any patient-record release.

What this route may not provide
DOPL enforces occupational licensing laws. It will not act as your attorney, recover monetary loss, settle a private dispute or conduct an investigation for your lawsuit. A licensing complaint should describe the professional or pharmacy conduct separately from a demand for repayment.

Do not guess about confidentiality
DOPL says complaints are not public information. Nevertheless, the form consents to disclosure of the complaint and supporting documents for investigation and states that records are governed by Utah’s records law. Anonymous acceptance and protection from identification to the subject were not verified.
