Authority and scope

Check the license and concern

The Board of Medical Practice’s current complaint form covers MDs and physician assistants, as well as podiatrists. Osteopathic physicians (DOs) have a separate Board of Osteopathic Physicians and Surgeons within the Office of Professional Regulation (OPR). The MD/PA instructions below must not be applied to a DO complaint.

Checked steps and filing details to confirm

  • For an MD or PA, use VtBMPOnline as a guest, registered user or licensee; registration is not required. Separately email the two required authorization forms to ahs.vdhmedicalboard@vermont.gov.
  • Alternatively, complete the complaint PDF, Authorization to Disclose PHI, and Authorization for Release of Medical Records. Email the signed package or mail it to Board of Medical Practice, 280 State Drive, Waterbury, VT 05671-8320. Digital signatures are accepted.
  • Give the professional’s name, detailed concern and supporting copies. The patient or legally authorized representative completes the releases; additional representative forms are linked. For DO instructions, contact OPR at 802-828-1502; its current submission requirements were not verified.

Limits

What this route may not provide

The Medical Practice Board can investigate unprofessional conduct and impose reprimands, restrictions, suspension or revocation. The checked materials do not establish a patient compensation remedy. OPR’s DO-specific complaint and remedy instructions could not be read during this check.

Anonymity

Do not guess about confidentiality

For MD/PA complaints, the release permits disclosure of the complaint, attachments and relevant records to the practitioner and Attorney General. The complaint form warns that patient and complainant information may become public in formal discipline. Anonymous acceptance was not established. DO anonymity and disclosure rules remain unverified.

Complaint timing

A filing deadline was not established for either route.

Ask the relevant board promptly. The Medical Practice process document describes investigations lasting months and contested proceedings taking longer; it does not promise completion. DO timing was not verified.

Process

What may happen after filing

Medical Practice investigative committees gather records and responses and assess possible misconduct. The full Board may approve closure, a negotiated disciplinary order, or proceedings after charges. The separate OPR investigation process was not verified.
