The Board regulates dentists, dental hygienists, Dental Assistant II registrants, faculty and temporary credentials, and relevant sedation permits. It also lists mobile dental facilities and portable dental operations. Identify the person and exact credential; ordinary practice ownership or an insurer’s coverage decision is not itself a professional violation.

How the official source says to file
- File online or complete the printable form. Give the professional’s full name and license type, patient details, incident dates and places, witnesses and a detailed factual account. Attach copies rather than originals.
- Return the paper form by email to enfcomplaints@dhp.virginia.gov, fax 804-212-2174, or mail/in person to Enforcement Division, 9960 Mayland Drive, Suite 300, Henrico, VA 23233-1463. For verbal filing or assistance call 800-533-1560 or 804-367-4691.
- Read the form’s declaration and complete its signature/date section as applicable. Save your own complaint copy before filing: DHP says it cannot provide one afterward, and a submitted complaint cannot be withdrawn.

What this route may not provide
DHP investigates possible law or regulation violations and may discipline a credential. It cannot order a refund, award damages or provide other personal remedies. It offers no immediate intervention with a practitioner.

Do not guess about confidentiality
Anonymous complaints are accepted, but DHP cannot guarantee anonymity. The form warns that information may be shared with the practitioner. To file anonymously, omit identifying details from the form, attachments, envelope and email. Missing patient names or incident details can limit investigation; disciplinary proceedings are public.
