Check the license and concern
The Board covers pharmacists, interns, technicians and trainees, pharmacies, nonresident pharmacies, outsourcing facilities and specified dispensing businesses. DHP Enforcement investigates both regulated individuals and pharmacy facilities. Identify the business and involved professionals; a complaint about a pharmacy is different from a general hospital or clinic complaint.

How the official source says to file
- Use DHP’s online complaint or printable form. Written complaints may be emailed to enfcomplaints@dhp.virginia.gov, faxed to 804-212-2174, mailed or delivered to Enforcement, 9960 Mayland Drive, Suite 300, Henrico, VA 23233-1463. Verbal reports or form requests: 800-533-1560 or 804-367-4691.
- Identify the professional’s full name and credential, business, patient, dates, places, witnesses and specific acts or omissions. Explain any injury, attempted resolution and reports to other agencies. Attach copies rather than originals.
- Read and sign the paper truthfulness declaration unless pursuing anonymous filing; ask staff how that option affects required fields or records authorization. Save a complete copy: DHP cannot supply one after submission, and the complaint cannot be withdrawn.

What this route may not provide
DHP may pursue licensing discipline but cannot require a refund, reimbursement, damages or another personal remedy. Explain the pharmacy or practitioner conduct separately from the financial result you want.

Do not guess about confidentiality
Anonymous reports are accepted, but investigation may be limited. The form warns that your information may be shared with the subject. Identification in an envelope, email or attachment defeats anonymity; DHP cannot guarantee it. Disciplinary notices and orders are public.

What may happen after filing
Enforcement gathers information and refers it to the Board. Insufficient cases close; supported allegations may proceed through an informal conference, consent resolution or formal hearing to possible license sanctions. The complaint source receives status or disposition notice and may need to testify.
