Washington has distinct disciplinary authorities. The Dental Commission covers dentists, dental assistants, expanded function dental auxiliaries, dental anesthesia assistants and state-licensed dental therapists. The Secretary of Health handles dental hygienists; denturists have their own Board. These credentials use the Department of Health (DOH) complaint intake. Identify the individual and credential, not simply a clinic brand.

  • Use the complaint page’s online option or the healthcare-provider paper form. Include the professional’s full name, profession, address, treatment/incident dates, factual details and witnesses; identify the patient and your relationship.

Limits

What this route may not provide

DOH generally excludes ordinary billing and insurance disputes unless fraud is suspected, and does not award malpractice compensation. RCW 18.130.160 nevertheless permits refunds of fees billed to and collected from consumers as a disciplinary sanction. Repayment is not guaranteed. Ordinary dental offices and administrative staff are not automatically licensed facilities or providers.

Anonymity

Do not guess about confidentiality

Anonymous complaints are possible but receive no follow-up. The form says providing your name makes the complaint identified even if you request anonymity, and names generally cannot be withheld under public-records law unless a whistleblower exemption applies. Disciplinary files are public subject to lawful redactions; patient-record protections do not guarantee complainant anonymity.

DOH states no statutory complaint-filing limit; report promptly.

DOH’s FAQ says there is no statutory time limit to file, but unavailable evidence can prevent action. Processing can range from weeks to two years. Those statements do not determine civil-lawsuit deadlines or promise resolution.
