Check the license and concern
The Commission handles pharmacists, interns, technicians, assistants, pharmacies and pharmaceutical firms. Washington licenses hospital and nonresident pharmacies. An out-of-state pharmacy shipping prescriptions into Washington may hold a Washington facility license even when its dispensing pharmacist does not hold a Washington individual license. Identify both the facility and worker, and ask intake to confirm jurisdiction.

How the official source says to file
- Use DOH’s healthcare provider/facility complaint form. Fill in individual-provider and facility information as applicable, patient details, dates, locations, a specific narrative and relevant supporting copies. Ask intake if the facility category is unclear.
- Email hsqacomplaintintake@doh.wa.gov, fax 360-236-2626, or mail Complaint Intake Unit, P.O. Box 47857, Olympia, WA 98504-7857. Send one copy. For email, use the message body or PDF/Word attachments, not cloud-storage links or services requiring a login.
- Keep the complete submission. An investigator may request a patient-record release or testimony. For help, call 360-236-2620.

What this route may not provide
Ordinary fee and insurance disputes generally fall outside disciplinary review, with suspected fraud a possible exception. Discipline can include a refund of fees billed to and collected from a consumer under RCW 18.130.160. This is discretionary and does not provide malpractice damages or guarantee repayment.

Do not guess about confidentiality
Anonymous complaints receive no follow-up. Providing your name makes the complaint nonanonymous, even if you request privacy. The form says complainant names generally cannot be withheld under public-records law unless a whistleblower exemption applies. Protected patient information is redacted from released disciplinary records.

DOH says no statutory provider-complaint limit; report promptly
DOH’s health-professions guidance says there is no statutory complaint-filing limit, but missing records can prevent action; report promptly. This is not a statement about court deadlines. Pharmacy-facility permit questions should be confirmed with intake. Review duration varies.
