- For the medical board, patients or legal representatives complete a separate Complaint Questionnaire for each provider, sign/date it, and complete the medical-information authorization. Include representative-status documentation if applicable and copies of relevant evidence. The package lists 101 Dee Drive, Suite 103, Charleston, WV 25311; call 304-558-2921 ext. 49867 for delivery questions.
- Other medical-board reporters submit a written statement identifying the practitioner and concerns to the Complaints Coordinator at that address.
- For the osteopathic board, use its online complaint form and supply the complainant, patient and licensee details and factual account. A patient must download the records release and have it notarized; the page permits uploading it or returning it by fax/email. Only the initial online fields were inspected.

What this route may not provide
The Board of Medicine excludes ordinary billing, insurance, business, personality and employment disputes and cannot prosecute crimes or litigate malpractice claims. Its possible remedies are professional discipline. The checked osteopathic intake does not establish a patient compensation remedy.

Do not guess about confidentiality
The medical-board questionnaire is sent to the practitioner. Its instructions say that after the final decision, the questionnaire and complainant identity become public, with specific patient medical information protected. Medical-board response and reply periods are 30 days each.
A filing deadline was not established for either board; ask promptly. The medical board gives the provider 30 days to respond and the complainant 30 days for an optional final reply. These are case-response periods, not incident-to-filing limits. No completion guarantee was established.
