West Virginia has separate nursing regulators. The RN Board's complaint process covers registered nurses, advanced practice registered nurses and dialysis technicians. The Practical Nursing Board handles licensed practical nurses. Match the person's credential to the correct Board before filing; an RN complaint portal is not the LPN route.

- Use the license-specific complaint portal above. For RN/APRN/dialysis technician matters, the RN Board also lists rnboard@wv.gov and 5001 MacCorkle Avenue SW, South Charleston, WV 25309.
- Identify the practitioner and describe the alleged conduct with dates, places and witness information. The RN Board emphasizes specific details, particularly for anonymous reports.
- For LPN filing questions, the Practical Nursing Board lists 304-558-3572 and lpn.board@wv.gov. Its own website supplies the LPN complaint portal and disciplinary-process brochure.

What this route may not provide
The RN Board lists nursing-law and professional-conduct matters, including unsafe care, impairment and falsified documentation. It excludes employment/personnel disputes, personality conflicts, domestic matters, mandatory overtime and wrongful discharge. This guide does not establish a compensation remedy through either Board.

Do not guess about confidentiality
Both Boards describe accepting anonymous allegations when sufficient information supports investigation. The RN Board treats investigation files as confidential but generally provides the licensee a complaint copy; the LPN brochure likewise describes disclosure to the licensee for response. Anonymous submission and confidentiality are different protections.

Ask the appropriate Board promptly.
This guide does not establish a deadline for filing a nursing complaint. Ask the Board promptly about the rules that apply to your situation. The RN Board's response period for an investigated licensee is not a public complaint-filing deadline.

What may happen after filing
The Boards review allegations, seek the practitioner's response and gather evidence. Review can result in closure or a disciplinary matter resolved by agreement or hearing. The RN Board publishes final disciplinary orders; an allegation itself is not a disciplinary finding.
