Check the license and concern
DSPS lists pharmacists, pharmacy technicians, in-state pharmacies and out-of-state pharmacies as Health credentials. Identify each person or entity involved, including the pharmacy location. Ask intake how to identify an intern or other worker whose credential is unclear, and distinguish pharmacy conduct from an insurer’s coverage decision.

How the official source says to file
- Use the Online link under Methods and select the Health complaint category. Alternatively, complete the paper form and mail to DSPS, Division of Legal Services and Compliance, P.O. Box 7190, Madison, WI 53707-7190.
- Name the people/entities, patient, dates, locations and witnesses; explain attempted resolution and other proceedings. Attach supporting copies, including relevant prescription evidence. Sign and date the form, and preserve evidence after filing.
- For healthcare complaints, complete the records authorization for each relevant provider/facility. It covers all health records, including prescription, psychiatric, substance-use and HIV information, and treatment discussions. A legally authorized representative must identify their authority. Missing authorization can prevent investigation.

What this route may not provide
DLSC investigates possible credential violations and can seek discipline. It does not recover funds for dissatisfied consumers. Its complaint route should not be treated as a refund, damages or insurance-appeal process.

Do not guess about confidentiality
Anonymous complaints are accepted, but proof may be harder and confidentiality is not guaranteed under open-records law. The subject usually receives your name. The release permits lawful disclosure in proceedings or public-record requests; information already obtained can be used after expiration or revocation.
