  • Download the complaint package linked from the official page. Complaints must be written. Identify the physician or PA, the patient and your relationship, care and hospitalization dates, treatment locations and other involved practitioners.
  • Describe the medical condition and the facts of your concern in detail. Attach relevant documentation and describe any previous efforts to resolve the matter. Preserve copies for yourself.
  • Complete the attached Authorization for Release of Medical Records and mail the package to Wyoming Board of Medicine, 130 Hobbs Avenue, Suite A, Cheyenne, WY 82002. The Board does not handle lawsuits for damages, ordinary billing disputes, disability-compensation questions, rudeness or requests for an apology. Its authority concerns professional licenses, with possible education requirements, restrictions, probation, suspension or revocation.
