- Download the Board’s complaint form. Give the pharmacy and professional names, location, incident date/time, detailed description and your contact information. Attach additional pages if necessary and retain a full copy.
- Sign the truthfulness and testimony declaration before a notary. The website expressly requests a notarized complaint, and the form includes a notarial certificate. Signing also agrees to testify if required.
- Confirm delivery with 307-634-9636 before mailing or delivering. The form still says 1712 Carey Avenue, Suite 200, but the homepage announces the move to Herschler Building East, 2nd Floor, 122 W. 25th Street, Cheyenne, WY 82002. Ask staff about patient-record releases and other filing methods.
