Authority and scope
Check the license and concern
DPR lists clinical psychologists and prescribing psychologists, professional and clinical professional counselors (LPC/LCPC), social workers and clinical social workers (LSW/LCSW), and marriage/family therapists and associate marriage/family therapists. Its professional directory also lists behavior analysts. The general DPR complaint instructions apply to individuals and entities regulated by that division. A separate addiction-counselor certification complaint route was not established; check the provider’s underlying Illinois license and any certificate issuer.

Match the route to the credential
Use the license listed in the practitioner’s records or official license lookup. A psychiatrist is a physician; psychiatric nurses have nursing credentials. Some psychologists have prescribing authority, so medication alone does not identify the licensing board.

- File in writing through the official DPR online complaint route or mail its paper complaint form as directed. The form does not accept telephone or walk-in complaints. Use a separate complaint form for each profession involved.
- Identify the respondent and profession, treatment dates, what happened and supporting documents. Send copies rather than originals. Keep the individual practitioner’s conduct distinct from a clinic, insurer or employer dispute.
- The complaint page gives placeholders for unknown respondent telephone and street details; follow those current directions rather than inventing information. For help contact the Complaint Intake Unit at 312-814-6910.

What this route may not provide
DPR investigates possible professional-law violations. The paper instructions say it cannot represent you in court or provide legal advice. Specific authority to obtain a refund or compensation for these credentials was not established; do not count on a licensing complaint to recover money.

Do not guess about confidentiality
DPR says complaints and investigations are confidential under its governing statute, with legal and agency-sharing exceptions. Anonymous acceptance and whether your identity or particular therapy records will be given to the respondent were not established. Confidentiality is not a guarantee that the practitioner cannot learn who complained.

Ask promptly about filing time limits
A filing deadline was not established by this check; ask promptly. The checked instructions do not establish a completion date or a separate behavioral-health incident-to-filing window.
What may happen after filing
The paper instructions say an acknowledgment and case number follow receipt and opening of the complaint. The precise investigative sequence and available sanctions for each credential were not independently established in this check.
