Psychologists have a separate board from the Behavioral Health and Human Services Licensing Board. The latter covers mental-health counselors and associates, social workers and clinical social workers, marriage/family therapists and associates, and addiction counselors and clinical addiction counselors, including associate levels. Behavior analysts have another board. The AG licensing-enforcement form expressly lists psychology, behavioral health/human services and behavior analysis, confirming shared intake for these categories.

Match the route to the credential
Use the license listed in the practitioner's records or official license lookup. A psychiatrist is a physician; psychiatric nurses have nursing credentials. Some psychologists have prescribing authority, so medication alone does not identify the licensing board.

- Use the AG Licensing Enforcement complaint route or its signed paper form. Prepare the respondent’s name, license number and mailing address; the webpage warns missing identifiers may prevent investigation, although the form labels license number “if known.” Add available phone/email, dates, location and a factual account.
- For mental-health complaints the form asks for patient identity and birth date, and whether treatment involved a court appointment or custody evaluation. Supply relevant supporting copies and identify witnesses. Do not include Social Security numbers.
- Sign and date the declaration and information-release consent. Mail the paper form to AG Consumer Protection Division, Indiana Government Center South, 5th Floor, 302 W. Washington Street, Indianapolis, IN 46204. Ask about any additional patient or representative authorization.

What this route may not provide
This licensing route investigates individuals and is not your private attorney. It cannot obtain money damages or bring criminal charges through the licensing process. The form says a complaint about a healthcare facility cannot proceed without identifying an individual practitioner; a facility issue may require another route.

Do not guess about confidentiality
The AG does not accept anonymous complaints; its page suggests contacting PLA about filing on your behalf, warning that this can limit investigation. The form states a complete complaint copy goes to the respondent. Other investigative information is generally confidential; the consent also permits information use and referrals connected with handling the complaint.

Ask promptly about filing time limits
