Care first. Seek urgent medical or dental attention for serious or worsening symptoms. Researching or preparing a complaint should never delay treatment.

Separate Kentucky boards for psychology, professional counseling, social work, marriage/family therapy, and alcohol/drug counseling

Four board-specific forms checked; social-work route and privacy checked, later form requirements limited. Some filing instructions were checked, but complete instructions for every listed credential have not been verified. Read the limits for your route.

Authority and scope

Check the license and concern

Kentucky does not have one therapist board. Use the Psychology Board; Licensed Professional Counselors Board for licensed professional clinical counselors and counselor associates; Social Work Board, including clinical social work; Marriage and Family Therapists Board; or Alcohol and Drug Counselors Board for the corresponding state credential. Select the board using the actual license or certification, not the treatment topic.

Match the route to the credential

Use the license listed in the practitioner’s records or official license lookup. A psychiatrist is a physician; psychiatric nurses have nursing credentials. Some psychologists have prescribing authority, so medication alone does not identify the licensing board.

Submission

Checked steps and filing details to confirm

  • Psychology and MFT: complete the appropriate signed form and patient records release for treatment complaints; mail to that named board at PO Box 1360, Frankfort, KY 40602. The releases include patient or parent/legal-guardian signatures for minors.
  • Professional counseling: use DPL-LPC-011, sign, attach supporting copies and witnesses, and complete the applicable medical/client-records authorization. Mail to its board at the same PO box or email lpc@ky.gov. The instructions require secure encryption when transmitting confidential information electronically.
  • Alcohol/drug counseling: the form requires a handwritten signature and allows mail to PO Box 1360, Frankfort, KY 40602, or email adc@ky.gov. Its header shows a conflicting ZIP; the submission instructions and form body use 40602. Ask before mailing if uncertain.
  • Social work: use its separate online complaint entry. The later form’s signature and records-release requirements were not established; ask the board at 502-564-2350.

Limits

What this route may not provide

The psychology, counseling, MFT and social-work instructions describe licensing discipline, including possible loss of a license. Specific refund or restitution authority was not established. A complaint about an employer, treatment facility or private certificate may require a separate route.

Anonymity

Do not guess about confidentiality

Professional counseling expressly rejects anonymous complaints and cannot withhold your name. Psychology, MFT and social-work instructions also say the practitioner learns the complainant’s name. These four routes warn that formal charges/action can make most investigative material public; patient records usually can be protected. The addiction form requires identification/signature, but its anonymity and public-record rules were not established.

Complaint timing

Ask promptly about filing time limits

A filing deadline was not established by this check; ask promptly. Professional counseling has an important later deadline: reply within seven days after receiving the respondent’s response. Its two-business-day acknowledgment and respondent’s 20-day response period are not patient filing limits.

Process

What may happen after filing

Psychology, counseling, MFT and social-work boards review possible legal violations and may investigate, dismiss, settle or hold a hearing. Witness testimony may be required. Addiction-board processing details were not established.

Evidence

Prepare a reviewable submission

State the concern you want reviewed, give a short chronology and factual consequences, and explain your request. Keep supporting records and follow the receiving authority’s instructions before attaching or sending them.
