Authority and scope
Check the license and concern
Ohio’s eLicense complaint selector expressly includes the Counselor, Social Worker & MFT Board and the Psychology Board. These are separate authorities: use the former for professional counseling, social work including clinical practice, and marriage/family therapy, and the latter for the psychologist’s credential. The Chemical Dependency Professionals Board is separately identified in IC&RC’s directory, with LCDC II, LCDC III, LICDC and clinical-supervisor endorsement credentials. It was not listed in the checked shared complaint selector.

Match the route to the credential
Use the license listed in the practitioner’s records or official license lookup. A psychiatrist is a physician; psychiatric nurses have nursing credentials. Some psychologists have prescribing authority, so medication alone does not identify the licensing board.

- Select the correct CSWMFT or Psychology Board in eLicense. Complete required fields and identify the person complained about, license type/number, contact information, incident location and your relationship to the practitioner.
- Describe what happened in detail, supply event dates and add relevant supporting documents. The form specifically asks for an end date. Keep copies of the account and documents you provide.
- The page includes patient name and date-of-birth fields and relationship choices such as client, parent/guardian and family member. These choices do not establish permission to release another person’s therapy records. Ask the selected board about consent, representative authority, signatures and any later authorization forms.
- For chemical-dependency credentials, contact that Board for the current complaint route. Its directory contact is 614-387-1110 or info@ocdp.ohio.gov; these are inquiries, not verified filing methods. Do not choose an unrelated board in the shared selector.
- If the electronic service is inaccessible, ask the correct board for an accepted alternative. This check did not establish paper, email or fax complaint submission for these Ohio authorities.

What this route may not provide
The shared complaint page concerns professional practice, not problems applying for or renewing a license. Its instructions do not establish every board’s scope, compensation powers or rules for unlicensed practice. Exact school-psychology or behavior-analysis coverage was not established. For a psychiatrist (MD/DO), use the medical guide; for a psychiatric advanced-practice nurse, use the nursing guide. Identify any facility or program complaint separately from the individual credential.

Do not guess about confidentiality
Anonymous acceptance, identity disclosure to the practitioner, public-record rules and treatment-record protections were not established for these boards. Do not infer confidentiality from the online form’s patient fields or general website privacy notice.
A filing deadline was not established by this check; ask promptly.
A filing deadline was not established by this check; ask promptly. No board-specific investigation estimate was verified. The event-date fields are not a published incident-to-filing limit.
What may happen after filing
The shared page says submitted practice concerns are reviewed to determine appropriate action. It does not establish the selected board’s investigative steps, hearing procedure, available sanctions or outcome-notification schedule. Chemical-dependency intake and process remain separately limited.
