Authority and scope

Check the license and concern

RIDOH’s psychology, social-work, marriage/family and mental-health therapy, and chemical-dependency professional pages each direct consumers to the Department’s common complaint service. The named boards remain distinct: Psychology; Examiners in Social Workers; Mental Health Counselors and Marriage/Family Therapists; and Chemical Dependency Professionals. The therapy page includes MFT and mental-health counselor associate licenses. Use the person’s actual state credential, including clinical social work or licensed chemical-dependency practice, rather than relying on the label therapist.

Match the route to the credential

Use the license listed in the practitioner's records or official license lookup. A psychiatrist is a physician; psychiatric nurses have nursing credentials. Some psychologists have prescribing authority, so medication alone does not identify the licensing board.

Submission

How the official source says to file

Open the linked Licensee Complaints service and search for the practitioner. A portion of a surname or business name can be used if spelling is uncertain. For names with apostrophes, the service suggests trying both versions.
Select the result and verify that it is the correct person or business before choosing File a Complaint in the next window. Match the license category to the relevant behavioral profession; a similar name is not enough.
Prepare the clinician’s identifying details, dates, locations, factual concerns and relevant correspondence. The checked entry instructions do not establish later signature, notarization, attachment or treatment-record release requirements. Follow the selected complaint form and ask the Complaint Unit about any uncertain requirement.
If filing for a child or another adult, ask what representative authority and consent are needed before supplying therapy records. Contact doh.ecomplaint@health.ri.gov for process questions; this check did not establish that address as an alternative filing method.

Limits

What this route may not provide

RIDOH excludes fee/billing disputes, personality conflicts and poor customer service from this process. A privately issued addiction or peer certification alone does not establish a RIDOH license. For a psychiatrist (MD/DO), use the medical guide; for a psychiatric advanced-practice nurse, use the nursing guide. Identify any facility or program complaint separately from the individual credential.

Anonymity

Do not guess about confidentiality

RIDOH describes investigations as confidential. It says the named licensee receives the complaint unless you request anonymity; make that request explicit. These statements do not guarantee that surrounding facts cannot identify you. The profession pages separately link public disciplinary-action information.

Process

What may happen after filing

RIDOH requests a licensee response and may seek testimony. Outcomes include closure, a concern letter or recommended discipline; contested allegations may proceed to an administrative hearing. The Department says a complaint cannot be taken back.

Evidence

Prepare a reviewable submission

State the concern you want reviewed, give a short chronology and factual consequences, and explain your request. Keep supporting records and follow the receiving authority’s instructions before attaching or sending them.
