Authority and scope

Check the license and concern

DOPL’s behavioral-health structure includes psychologists, clinical mental-health counselors, marriage/family therapists, social workers and master-addiction/substance-use-disorder counselors. Psychology credentials also include prescribing psychologists, school psychological practitioners and behavior analysts. Utah also regulates behavioral-health coaches and technicians. Match the exact credential; a prescribing psychologist is not automatically a physician.

Match the route to the credential

Use the license listed in the practitioner's records or official license lookup. A psychiatrist is a physician; psychiatric nurses have nursing credentials. Some psychologists have prescribing authority, so medication alone does not identify the licensing board.

Submission

How the official source says to file

Use DOPL’s online complaint option or its downloadable form. Complete each item, using N/A where appropriate. State the profession, individual or business, dates, locations, witnesses and your relationship to the matter.
Give a factual explanation and copies of supporting material; DOPL does not return submitted evidence. The form includes a truth declaration under criminal penalty, with name, date and place. Identify anyone acting as your representative.
The paper form lists DOPL Investigations, 160 East 300 South, PO Box 146741, Salt Lake City, UT 84114-6741. Ask Investigations at 801-530-6630 about the appropriate patient-record release or representative authority before sending sensitive third-party therapy material.

Limits

What this route may not provide

DOPL addresses professional licensing and unlicensed practice. It does not represent you, recover money, mediate private disputes, or investigate for your private litigation. Mental-health facility regulation is with Health and Human Services. Use medical/nursing guides for psychiatrists and psychiatric advanced-practice nurses.

Anonymity

Do not guess about confidentiality

DOPL says complaint information is not public information, but the form allows disclosures needed for investigation under applicable records law. That is not a promise of anonymity from the practitioner. Anonymous-filing requirements and a behavioral-health-specific records authorization were not established by this check.

Complaint timing

Ask promptly; investigations may exceed 180 days

A filing deadline was not established. The mental-health investigations guidance says some cases exceed 180 days. That describes agency processing, not permission to wait or a deadline for a court case.

Process

What may happen after filing

DOPL evaluates allegations and investigates potential licensing violations, including unlicensed mental-health practice disguised as coaching. It may resolve a matter informally or pursue formal discipline. Formal disciplinary actions are public; the mental-health guidance distinguishes private informal actions.
